| Effective 1/1/2011, Northwest Health Employees & Siloam Springs Memorial Hospital employees have a new TPA, MedPartners. This means a new claims address, new phone numbers for benefits, eligibility and precertification. The network is now PremierCare Northwest. You should have already received the new Account Info Sheet with all of this information at your office. Please contact Marci Miller if you did not receive this information. |
